Palliative Care brings an expertly trained team to address issues surrounding serious or chronic illnesses. This specialized team helps you, your family members, and your physician to set personal goals focused on improving quality of life in your personal healthcare journey. Common diagnoses include, but are not limited to, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), and Cancer.

Often times medical equipment is needed to help manage a chronic condition or recover from an injury, illness, or surgery. Whether it helps you breathe easier, sleep better, or makes your daily activities more effortless, we are committed to providing equipment that will help you wherever you are.
When an oral medication cannot do the trick your physician may order more effective medications which are delivered directly into your blood stream through an IV. Our Infusion Pharmacy team is available to provide these medications at home to reduce your need to seek daily treatment at a facility. We also provide nutritional formulas ordered by your physician to enhance your body’s dietary needs at home. We utilize nurses and the latest equipment to help you administer your own treatments which gives you greater schedule flexibility and makes care convenient.
In addition to inpatient dialysis treatment and seven dialysis centers, Intermountain Healthcare offers dialysis at home services to patients needing treatment for End Stage Renal Disease (ESRD). Dialysis at Home gives patients the flexibility to find the best care environment for their dialysis needs.

Primary Care at Home
Committed to house calls, our doctors and providers drive to patients where they live. This home-based medical care is designed to help chronically ill patients stay well and stay home. A multidisciplinary team that collaborates with our patient’s health plan to care for the whole patient, bringing medical care, behavioral health, palliative care and social support services to patients in the comfort of their own home—wherever and whenever they need it. This will be initially offered to Medicare Advantage insured but may expand in the future.
Acute Recovery Care at Home
It is like you are in the hospital but while sitting in bed at home. Our providers bring evidence-based medicine to home-based acute inpatient care with 24/7 coverage from doctors, nurses, and other medical team members. The doctor will oversee your care from your own home instead of the hospital. You will have a dedicated Recovery Care Coordinator who will be available to you 24/7. A registered nurse will provide care to you in the comfort and convenience of your home. This care is best suited for conditions that only need a short stay, usually lasting three to five days.
