Primary Care at Home

Committed to house calls, our doctors and providers drive to patients where they live. This home-based medical care is designed to help chronically ill patients stay well and stay home. A multidisciplinary team that collaborates with our patient’s health plan to care for the whole patient, bringing medical care, behavioral health, palliative care and social support services to patients in the comfort of their own home—wherever and whenever they need it. This will be initially offered to Medicare Advantage insured but may expand in the future.

Acute Recovery Care at Home

It is like you are in the hospital but while sitting in bed at home. Our providers bring evidence-based medicine to home-based acute inpatient care with 24/7 coverage from doctors, nurses, and other medical team members. The doctor will oversee your care from your own home instead of the hospital. You will have a dedicated Recovery Care Coordinator who will be available to you 24/7. A registered nurse will provide care to you in the comfort and convenience of your home. This care is best suited for conditions that only need a short stay, usually lasting three to five days.
